Duplicate comments shouldn't be a problem now, if your new comment is identical to your last comment it won't post.

With posts it's how the pagination works, it works out the posts to show then someone posts a new post and throws it all out by one so then it looks like posts have doubled up.

It's horrible legacy code, the app doesn't have those issues though.
